From then on, Perksworth does the chasing. When a client books, the appointment isn't confirmed until the form is signed. When someone joins as a member, any forms they still need are sent to them automatically. Nobody slips through unsigned, and every signed form is kept as a PDF you can pull up any time, showing who signed and when.
What you can do
Build your own forms, no developer needed
Build the form you need from ready-made fields: text, phone, email, number, date, drop-down lists, tick boxes, consent and signature. Choose the type of form (a plain form, a waiver, a consent form or a new-client intake form), and Perksworth knows how to handle it.
Publish and keep every version
Forms start as drafts, then you publish them, and later you can archive them. Only published forms can be required. If you change a form, past answers stay exactly as they were signed, so later edits never change old records.
Signed before they book
Make a form required before booking, just for the services that need it, so only your tattoo, treatment or higher-risk services ask for a waiver. The appointment isn't confirmed until it's signed, unless the customer has already signed a valid copy.
Forms for new members
When someone becomes a member, they're automatically asked to fill in any required joining forms they haven't already done, so new members are set up properly from day one.
A link customers can fill in on their phone
Every published form has a link you can share, so clients can fill in their intake form or waiver before they've even logged in. If they tap submit twice, you still only get one copy.
Sign by typing their name
Clients type their name to sign, and the time and their internet address are saved. Once signed, the form is locked and can't be signed again. Forms without a signature are complete as soon as they're sent.

Every signed form can be downloaded as a PDF, with accents and other languages shown properly.

Decide whether forms must be signed before booking, and whether a signature lasts forever, runs out after a set number of days or is needed every visit. You can also make sure every waiver has a signature box before it goes live.

Questions
- Can a customer book without filling in the form?
- Not if you've made it required. When someone books, Perksworth checks for any required forms they haven't done, and the appointment isn't confirmed until they're signed. You can choose which services need which forms.
- What record is kept of each signature?
- Each signed form is locked and records the signer's name, when they signed and their internet address. You can download it as a PDF any time.
- Do clients need an account to fill in a form?
- No. Every published form has a link you can share, so customers can fill it in on their phone before logging in. If the form has no signature, it's complete when they send it. If it does, they sign to finish.
- How long does a signed form last?
- You decide. A signature can last forever, run out after a set number of days, or be needed every visit.
- What happens if I change a form after clients have signed it?
- Nothing they signed changes. Each signed form keeps the exact version the client filled in, so your records always show what they actually agreed to.
